Polio resurgence raises alarm amid new outbreaks

Polio is back in the spotlight due to recent outbreaks, including a notable resurgence in Pakistan. This issue was highlighted over two years ago when a case of paralytic poliomyelitis was detected in New York, and the virus was found in wastewater across several counties. Though less than 1% of infections lead to paralytic polio, wastewater evidence indicates broader infection.

Globally, vaccine-derived polio outbreaks continue to be a concern, such as the recent outbreak in Gaza. This has led wealthier nations to adopt inactivated polio vaccines. These vaccines are effective only with high uptake rates and when polio is nearly eradicated.

The Global Polio Eradication Initiative (GPEI) prefers the oral polio vaccine for its superior mucosal immunity, ability to spread from vaccinees to close contacts, ease of administration during large campaigns, and cost-effectiveness.

Currently, there is an alarming outbreak of wild-type polio in Pakistan—the first in several years. Islamabad has reported a single case and wastewater detections suggest ongoing transmission. Afghanistan also faces multiple cases, both wild-type and vaccine-related.

Efforts to eradicate both types of polio hinge on widespread vaccination. However, immunization rates have declined in Pakistan partly due to historical distrust stemming from a fake vaccination program used by the U.S. government during the hunt for Osama bin Laden.

Recently, the Taliban banned most of Afghanistan's polio vaccination program, exacerbating concerns about controlling the virus. Effective vaccination programs require minimizing political interference and addressing public concerns promptly.

Pakistan is currently conducting an extensive vaccination campaign while global observers hope for quick containment. Afghanistan's pause on its efforts presents additional challenges.

Eradicating polio will necessitate global cooperation; no single nation can achieve this alone.
